Dear Parents and Guardians,
Welcome to another wonderful year of learning at Cottonwood-Oak Creek School District! My name is Bethany Campista, and this will be my second year as your National School Lunch Program Coordinator. It’s my responsibility to make sure that the Applications for Free and Reduced Price School Meals are proccessed correctly for our District. When you fill out the mandatory steps on the application, it speeds up the proccessing of your application. Below, I have provided you with a checkbox of these mandatory steps to make sure your application is filled out correctly and proccessed in a timely manner.
□ Names of EVERY child (under the age of 18) in the home
□ Every Adult’s income □ Total household members (this
□ If child is a foster, homeless, migrant, or runaway student check corresponding box
□ Every Child’s income totaled together (if any) number MUST match amount of names on the application.
□ Case number of SNAP, TANF, or FDPIR if applicable (AHCCCS numbers do not qualify)
I want to make sure all our families know that applications HELP our schools!!! Whether or not you believe your family qualifies for this service, we ask that
you take a moment to complete the application correctly. You’re information
will be added to a percentage for your school. That percentage is used to apply
for valuable state and federal funding opportunities for ALL of our students.
• A quick reminder: You must fill out a new Application for Free and Reduced Price Meals every school year. Last school year’s application expires on September 15th, 2016. Please fill out, and return the completed application before this date, or your student’s status will return to Paid price.
• If your child is new to our District please highlight, circle, or star their name. This will help me to know that they do not have an application from last year, so I can process the application promptly.
• We all know that financial situations change. If you do not qualify for Free or Reduced Price meals at the beginning of the school year, please feel free to fill out a new application at ANY time during the school year.
• If your child(ren) owe(s) money prior to being approved for a free or reduced status, this balance is still due and will be your responsibility.

Instructions for Applying

The Cottonwood-Oak Creek School District offers free developmental screenings for preschool children between the ages of 3 to 5 years who live in the school district and are not already receiving special education services or enrolled in Kindergarten. The screening are typically held on a monthly basis. The next screening is April 13, 2016. The screenings will check a child's speech, motor, cognitive, adaptive, and social development. For an appointment call 634-2288 ext 1129 or 1124. Arizona Early Intervention program also offers developmental screenings for children age birth to three years.
